Street Medicine

San Francisco Community Health Center’s Street Medicine Team is currently comprised of:

• A dual certified primary care and psychiatric nurse practitioner

• A registered nurse case manager

• A medical assistant/phlebotomist

• A community ambassador.

The team offers street and walk-in based primary and psychiatric care to unhoused individuals, as well as specialty referrals, HCV treatment, wound care, MAT and HIV care. The team currently goes out on street rounds Tuesday and Thursday afternoons and offers a walk-in clinic on Fridays from 1–3:30 pm.

Recognizing the variety of barriers that unhoused people may face with accessing care, our team remains committed to meeting patients where they are—on the street, in tents, cars, and in encampments. Some examples of the services provided during routine visits include:

• Blood draws

• Vaccinations

• Administration of GAH injections

• Management of weekly to monthly medication drop-offs.

Our team members dispense harm reduction supplies, including Narcan, and are equipped to respond to overdoses. As an extension of SFCHC’s clinical services, the Street Medicine Team believes in nonjudgmental, whole person care and operates based on a harm reduction model.
