Wellness Clinic

The Wellness Clinic is a core function of San Francisco Community Health Center. In 2015, we became the first new Federally Qualified Health Center (FQHC) in San Francisco in 40 years, enabling us to expand our capacity and provide full primary care services. The Wellness Clinic works in close collaboration with the Street Medicine team, Trans Thrive, Taimon Booton Navigation Center, and our other sites to coordinate primary care for patients accessing SFCHC services in multiple settings. By providing medical services that are centered around the needs and circumstance of our community, we reach thousands of patients many clinics cannot.
