TransLink
TransLink emerges as a beacon of empowerment and support for a demographic often facing systemic challenges. By tailoring outreach and services to the unique needs of homeless and unstably housed transgender women aged 25 and up, TransLink aims to not only reduce the prevalence of HIV, viral HEP-C, and substance use but also creates a platform for community building and resilience. The initiative's commitment to comprehensive care, reflects a holistic approach that goes beyond the traditional healthcare models.
This peer-focused initiative is designed to significantly reduce the risk of substance use, HIV infection, STI, and viral hepatitis infection among homeless and unstably housed transgender women, ages 25 and up who live in San Francisco.
We aim to meet these objectives by targeted, tailored outreach and specific services that engage the many communities that trans women of color are part of. Through engaging activities, we intend to bring transgender women together to create stronger, more unified, and more empowered systems for preserving health, overcoming stigma, and improve health, well-being, safety, and self-determination.
Participating in TransLink will give clients 6-months to address and create a pathway to live long, vibrant, happy, healthy lives. This 6-month period will include a comprehensive client assessment, care plan, and extensive navigation to vital resources and services. Each participant will be assigned a prevention navigator that will provide linkage to;
Comprehensive primary medical services to include:
Dental, Medical, Behavioral Health Care,
Treatment for substance use disorder,
Optional seeking safety groups,
Peer-based social support services,
PrEP/PEP access,
and Social/Recreational engaging activities.
Through its innovative strategies, TransLink stands as a promising force in addressing health disparities and contributing to the overall betterment of transgender women in the San Francisco Bay Area. TransLink represents a vital step towards fostering a stronger, more united community that promotes the vitality, well-being, security, and autonomy for Transgender women ages 25+."
